To stop email notifications from this form, simply login, go into your profile and select not to receive email notifications.

Might have to copy and past the URL into your address bar.  The form seems to be strange with urls.

Ill check into this.. I think we corrected this in a previous version, but the problem may have came back.  For now you could just put junk data in. 

Also, note we had a problem and lost the "problems section of the form".  We will continue to support the software, and are very sorry for the loss of some valuable information.
